Neurology

In our medical facility we deal with neurological issues in its entirety and mainly focus on following diseases in the central and peripheral nerve system: epilepsy, differential diagnosis of transient disorders of consciousness and collapse conditions, headaches, sleep disorders, movement disorders ((gait disorders, Parkinson’s disease, tremor, etc.), balance disorders, conditions after cerebrovascular accidents, neurodegenerative diseases, multiple sclerosis as well as impairment of peripheral nerves such as entrapment syndromes (e.g. carpal tunnel syndrome, etc.), polyneuropathy, impairment of nerve roots of vertebrogenic origin, neuromuscular disorders, muscular diseases, etc.

Within neurological outpatient treatment we perform EEG and EMG examination, motor evoked potentials and other modalities of evoked potentials (SSEP, VEP, BAEP) and lumbar puncture with cerebrospinal fluid examination. The examination can be supplemented within our facility with imaging examination (CT, magnetic resonance, ultrasound) and possibly further laboratory examinations. Given the generally limited capacity and time and economic demands of the electrophysiological examinations, we kindly ask indicating physicians for careful consideration of their indication according to Recommendation for EEG and EMG Indication.  In case of indication of surgical intervention we cooperate with neurosurgical clinics (Homolka Hospital, Central Military Hospital, Motol Hospital), we also cooperate with neurological clinic of the Military University Hospital and Centre for Sleep Disorders of National Institute of Mental Health. Thanks to the cooperation with our CT departments we provide our patients with nerve root syndrome selective periradicular injections or periradicular ozone therapy.

There are 6 physicians at your service in our neurology department, all of them have long-term experience and specialized expertise in the field of neurology:

MUDr. Petr Bušek, Ph.D., focuses on electrophysiological methods (EEG, EMG) and specialises mainly in epileptology, particularly with focus on differential diagnostics of seizure disorders during sleep. He simultaneously works at the Neurological Clinic of Central Military Hospital where he is in charge of detailed examination of the patients with diagnostically or therapeutically more complicated seizure disease (e.g. video-EEG monitoring, etc.). He cooperates with the Epilepsy Centre at Homolka Hospital in case there is a possibility of epilepsy surgery treatment.

MUDr. Jana Hofmannová, with long experience in electrophysiological methods, mainly in EMG and evoked potentials. For these purposes there is a modern machine available—EMG/EP Keypoint – Dantec, Walter.

MUDr. Zdeněk Konrád, with many years of experience in manual medicine, with focus on imaging methods of examination and treatment of musculoskeletal diseases.

MUDr. Ing. Ivana Patáková CSc.,with long experience, focusing on electrophysiological methods, mainly EEG examination. For these purposes there is a modern EEG machine – Walter, Wavepoint Headbox.

MUDr. Miroslava Sinkulová, with years of experience covering the full spectrum of neurological diseases, focuses on EEG examinations.

MUDr. Jana Zárubová, with years of experience, mostly in epileptology. She has clinical experience in pharmacological treatment of epilepsy, females with epilepsy (among other things counseling before a planned pregnancy, guidance during pregnancy and after giving birth), but also in complex care of people with epilepsy. In addition to Affidea Praha she also works at Neurology department in Thomayer Hospital where patients with unclear diagnosis can be examined with video-EEG or get hospitalized. She also cooperates with the Centre for epilepsy in Motol Hospital where she deals with epilepsy surgery and stimulation therapy. She is also an experienced electroencephalograper. Besides Affidea Praha and Thomayer Hospital she is also a chair person of the Epistop Civic Association which aims to improve the lives of people with epilepsy - ww.epistop.cz.

 


Epileptology

Epilepsies are a group of the most common chronic neurological diseases that are characterized by epileptic seizures of different clinical picture. The prevalence of the disease is high, according to different resources it is between 0.5-1%. Only in the Czech Republic there are 70.000 patients with epilepsy.

People with epilepsy need correct diagnosis, correct treatment, but also a lot of information about how to live with epilepsy so that it affects their quality of life as little as possible. They are diseases that are still covered in certain stigma, myth and embarrassment.

EpiStop Civic Association tries to help people with epilepsy. On www.epistop.cz website there is a lot of information about activities that should teach other people about epilepsy, so that they get rid of fear and worries about epileptic seizures and teach them how to give first aid correctly. There are free downoloadable materials for both patients and physicians, such as “Minimal Diagnostic and Therapeutic Standards”, „A woman and Epilepsy“,“ Motherhood and Epilepsy“, „Epilepsy in Adulthood – Basic Information“. There is also a discussion forum and social counseling on the website.

Our department is able in addition to regular outpatient practice also perform some specialised tests, e.g. for drivers. For serious conditions with the need for radical surgery we cooperate with neurosurgical sclinics (Homolka Hospital, Central Military Hospital, Motol Hospital, Vinohrady Hospital), we also cooperate with inpatient neurology departments at Institute of Aviation Medicine, Bulovka Hospital, Neurological clinic of Military University Hospital where there is video-EMG monitoring in sleep laboratory and specialised clinics.

Thanks to our CT department we are able to perform selective PRT examinations for patients with spinal diseases.
We also perform neurological examination with necessary supplements as a part of preventive programmes.

Motor evoked potentials

It is a non-invasive painless examination which is used in testing the function of motor pathways. The stimulation is performed by applying a magnetic coil under the surface of the skull or under neck or lumbar spine. The stimulations leads to a short muscle twitch in an upper or lower limb from which we get an answer by self-adhesive surface electrodes. The examination provides information about the malfunction of motoric pathways mainly in the central section, which means brain and spinal cord.

Preparations for examinations 

EEG

One day before or on the day of the examination wash your hair using regular shampoo. Do not use hair conditioner, hair spray or other hair products!

EMG

Before the examination wash the whole body using regular soap. Do not use any cream, gel, lotion or deodorant after that. It is recommended to bring a towel for the examination (80×35 cm).
